Reasons for nomination (in several sentences) *

Attach the reporting (series) in English (PDF format, 10 Mb maximum size). The word restriction is 15,000 words (in total). By attaching the reporting (series) you confirm it is original and is written by the author(-s) you nominate. You are aware that it may be posted on the website of the contest and in FJA annual report for non-commercial use *

Attach a signed copy of the author's (-s’) consent to participate in the contest if the application is submitted by the third person (PDF, 10 Mb maximum size). If there are two or more authors submit all consents in one PDF file

Indicate how the prize money will be distributed among the authors in case of win, i.e. Author 1 – 50%; Author 2 – 50% (if applicable)
